Blue Cards Instructions
Introduction
The Bluecard system requires a person working with children in Australia to have a bluecard. Any members of the church that will
be working with children will be required to hold a valid bluecard that is linked into the Hollandpark Church bluecard register.
Please see a bluecard coordinator or any leaders of the church if you are intending to be work with children in the church.

Australians
Online
For those with a driver license simply follow the steps for foreigners below using the driver license as the CRN.
Paper way
Alternatively, download and fill in an bluecard application form ( Blue Card Application Form). Have it witnessed and send the
form the bluecard coordinator. The bluecard coordinator will sign and send it for you.
Foreigners (not residing in Queensland)
These are the steps for non Australians:
NB. For requested postal address, please use the Holland Park Church postal address: PO Box 6, Holland Park, QLD 4121.
- Get a CRN (Customer Reference Number) by filling in the Remote CRN package: Transport and Main Roads Corporate Form s F5342 - New Customer/Photo and Signature Remote Application (for agency card)

After you have done that you will eventually receive a email about your CRN application. Proceed with those information to the next Step below.
- Register for a bluecard services online account: my.bluecard.qld.gov.au (my.bluecard.qld.gov.au ) using your CRN number from the email they sent to you.
- Tip: Use the church postal address: PO Box 6, Holland Park, QLD 4121
- Once your bluecard services account has been created, take note of it and send it the church bluecard coordinator with
these details:
bluecard a/c number
your full name
Date of birth
- The church bluecard coordinator will link your bluecard account to the church with those details.
- the bluecard system will send a notification to you.
- once that is done you can apply for a volunteer (free) bluecard with this link: https://my.bluecard.qld.gov.au/login
Tip: You will get a message like that if you are not linked yet to HP church: That is ok just try again after we have successfully
linked you.
Renewals
The documentation is in bluecard renewal
Renewal is alot simplier if you done it the online way. Simply login into my.bluecard.qld.gov.au and choose renew.
Othewise download and fill in form BlueCard application form using your driver license and have it witnessed and as usual send it the church bluecard coordinator.
Remote CRN form (for anyone not in Australia)
Evidence of Idenity (EOI)
When applying for a CRN, you will be required to provide 3 witnessed documents. Please refer to this https://www.qld.gov.au/transport/licensing/driver-licensing/identity
Tip: Category
For Category A: you may use Passport, Visa for travel to Australia
For Category B: you may your credit card debit card. But make sure the card number are not shown.
You can use 2 or category A and 1 of category B or 1 of category A and 2 of category B.
Tip: witness
Many of your documents will need to be signed in the presence of an approved witness. An approved witness is a:
- police officer
- medical practitioner
- consular or ambassadorial officer
- solicitor, barrister or judge
- Justice of the Peace or a Commissioner for Declarations
- Notary Public or a person authorised by law to witness and sign declarations.
Section 4: Residence Evidence
Residential Address - this refers to your current home address in the US or whatever home country is.
In Section 4 of the form, your evidence relating to your residential address (for example: driver licence, photo ID, lease/rental agreement, or a utility bill) needs to be witnessed by an approved person. (approved person as in witness).

Tip: In Section 5 of the form, please enter Blue Card Services as the agency:
